An entitled Central Government serving staff with residence within a 5 km radius of the CGHS Berhampur Dispensaries has to apply online for CGHS Berhampur membership as well as to send the application’s hard copy duly signed by the applier and permitted by HOO with age proof of dependants, residential proof, 2 copy stamp size photograph of all persons, one pasted on the form as well as attested by the HOO along with other necessary documents such as payslip to AD, CGHS Berhampur through HOO.

OFFICE MEMORANDUM

Consequent upon the merger of postal dispensary located at Cuttack and Berhampur with CGHS under the administrative control of Additional Director of CGHS, Bhubaneswar on and from 01/04/19, the above dispensaries have renamed as CGHS Cuttack and Berhampur, CGHS wellness center.

As per prevailing norm regarding jurisdiction of CGHS wellness center, which covers approximately 5km radius from a particular dispensary, the demarcation of the jurisdiction under Cuttack CGHS, Wellness Centre and Berhampur Wellness center has been finalized as per Annexures attached for these two wellness centers separately as mentioned under: –

  1. Annexure- I for Cuttack, Wellness Centre.
  2. annexure- II for Berhampur, Wellness Centre.

The administration of all government offices located in the city of Cuttack and Berhampur must compulsorily deduct CGHS contribution from the salary of their employees whose residential address falls within the jurisdiction of CGHS, Wellness Centre in those two cities and this is not linked to the application for CGHS cases by the concerned agency. 

Central Govt. pensioner residing within the area at the jurisdiction may avail the benefit of CGHS, but will not be eligible to draw fixed medical allowance with their pension as per rule.

  1. What are the revised Guidelines for Post-operative follow-up treatment at Empanelled Private Hospitals for critically ill CGHS beneficiaries?
  2. In the view of difficulties being faced by critically ill CGHS beneficiaries in getting post-operative follow-up treatment at CGHS empanelled hospitals, the matter was reviewed and it has now been decided that critically ill CGHS beneficiaries shall be permitted for follow up treatment in CGHS empanelled hospitals as per the details given Here.
  3. Can an accredited journalist with CGHS Card seek treatment in Private Empanelled Hospitals in emergency and get a credit facility?
  4. YES, they are eligible to avail CGHS rates at empanelled HCOs on a payment basis. There is no provision for reimbursement under CGHS.
